The gradient starts out quite gentle and increases as you approach the summit.
Down at Holyrood its a gentle slope as you approach the old brewery, but it then gets steeper as you reach the middle of the High Street.
The last part of the Royal Mile. from the City Chambers, past the Witchery and the Whisky Centre, up to the Castle Esplanade is quite a climb. Pushing a twin buggy was a challenge, so I would imagine it would not be easy to push an adult in a wheelchair.
It would present a difficulty for someone with a heart, or respiratorty condition.
